Pregnancy hormone predicts postpartum depression

Women with elevated levels mid-pregnancy at higher risk, study finds

Irvine, Calif., February 2, 2009
Women who have higher levels of a hormone produced by the placenta midway through pregnancy appear more likely to develop postpartum depression, a study authored by a UC Irvine researcher finds.

The discovery could help identify and treat women at risk for postpartum depression long before the onset of symptoms.

Ilona Yim, psychology and social behavior assistant professor, and colleagues found that women whose levels of corticotrophin-releasing hormone started to increase more rapidly around 25 weeks of gestation had a higher incidence of postpartum depression.

Normally secreted in very small amounts by the hypothalamus, this hormone regulates the body’s response to stress. During pregnancy, large amounts are produced in the placenta and are associated with delivery.

“The hormone we studied plays an important part in pregnancy and has been linked to depression,” Yim said. “Many factors may cause some women’s bodies to produce more of this hormone during pregnancy. Evidence suggests that stress early in pregnancy could play a role.”

The researchers studied the hormone-postpartum depression link using data from a larger study funded by the National Institute of Child Health and Human Development. They took blood samples from 100 pregnant women and assessed symptoms of depression throughout pregnancy, then again nine weeks after delivery.

Of the 100 women, 16 developed postpartum depression symptoms during follow-up visits. Three-fourths of those women, the study concludes, could have been identified in mid-pregnancy based solely on hormone levels.

In addition to Yim, Laura M. Glynn, Christine Dunkel Schetter, Calvin J. Hobel, Aleksandra Chicz-DeMet and Curt A. Sandman worked on the study, which appears in the February issue of Archives of General Psychiatry. A U.S. Public Health Service research award from the National Institute of Child Health and Human Development funded the study.

An increase in inflammation in women with asthma, rheumatoid arthritis, psoriatic arthritis, and appendicitis has been noted during late phase of menstrual cycle. In this study of 76 regularly menstruating (28 day cycle) adult women (mean age 31.5) with uveitis, an increase in incidence of acute anterior uveitis was noted during late stage of menstrual cycle. Late stage of menstrual cycle was defined as postovulatory, starting on day 17 of menstrual cycle. Some attacks were also noted on day 9-12 and days 13-16, but the greatest number of attacks occurred on days 25-29. 17 % were on oral contraception, but contraception therapy has same decrease in hormones on day 22 of pill pack as during the late stage of menstrual cycle. The authors postulated that the rapid withdrawal of the anti-inflammatory effects of estrogen and/or progesterone in the late luteal phase may explain the increase in inflammatory attacks during late menstrual cycle.

are you still in flare up of your uveitis? are you seeing an ocular immunologist to see if a steroid sparing approach can be implemented?

You will have to travel to NYC or to Boston to see an Ocular Immunologist. I think that DR Hinkle from MERSI in Cambridge, MA travels to upstate NY once a month but I can't recall which City. if you go to http://www.uveitis.org there is contact information for Dr Hinkle and DR Foster who are both with MERSI and the ocular immunology and uveitis foundation. you can email them for information on getting an appointment.
there are not many true ocular immunologists in the US or any other country but getting one of them for a consultation can get you to durable remission.

I knew that but couldn't recall it when I needed it for you so i'm glad you contacted MERSI and made the appointment. Hopefully your local doctors will be willing to work with whatever plan DR Hinkle develops.
Dr Foster used to travel to Albany but gave it up earlier this year. he is just too busy and is now 65 so we need to cut him some slack if you know what I mean.
